​Please Note that you must be seen in this office at least once a year to maintain an active status as a patient with this practice. This can typically be done with an Annual Wellness Exam or medication follow up appointment. If you are not seen in this office in three (3) years, you will be listed as inactive. If you have a chronic health condition or are on a high-risk medication, you will need to be seen in the office every three (3) to six (6) months as determined by the physician.
